Key Responsibilities Develop Patient Relationships: Perform telephonic outreach to patients after clinical events or diagnoses, fostering strong relationships and trust. Collaborate with Clinical Teams: Work closely with healthcare professionals, utilizing various resources across the health system to positively impact patient experiences. Research Community Resources: Proactively identify, examine, and leverage new community resources to assist patients effectively. Professional Network Participation: Engage in a growing professional network that is defining care guidance standards by disease state, contributing to industry advancements. Empower Patient Communication: Enable patients to express their preferences and priorities to their healthcare teams, promoting active participation in healthcare decision-making. Program Development: Continuously contribute to the development, implementation, and evaluation of the Care Guide navigation program as we expand. Address Non-Clinical Factors: Help address non-clinical factors significantly impacting patient health, thereby improving patient outcomes, satisfaction, and quality of care. Adaptability: Perform any other duties necessary to uphold our company values, fulfill our mission, and drive success. Qualifications and Skills Essential Qualifications: Technical Proficiency: Be proficient in using a personal computer and possess a technical and analytical skill set. Excellent Communication Skills: Demonstrate excellent communication, organizational, and interpersonal skills. Professional Experience or Education: Hold a Bachelor's degree or equivalent professional experience.
