Patient Care Coordinator – Part-time (100% Remote, Potential for Full-Time) Momentous Health provides online cognitive evaluations to geriatric patients across 42 states. We are seeking a Patient Care Coordinator to join our team. This is a remote role with a minimum of 20 hours per week and potential to expand into full-time for the right fit. In this role, you will serve as the first point of contact for patients and their families, guiding them through scheduling, intake, and follow-up with empathy and professionalism. You will also support our clinicians by managing referrals and ensuring accurate, timely communication with providers. Key Responsibilities: Patient Support & Scheduling • Schedule and confirm patient appointments. • Assist patients with technology setup for telehealth sessions. • Respond to patient inquiries via phone and email with compassion and professionalism. • Manage patient intake paperwork Referral & Provider Coordination • Manage incoming referrals from our partners. • Communicate with physicians and clinical staff. • Maintain accurate documentation of referrals. Administrative Support • Verify patient insurance coverage and submit claims accurately. • Maintain and update patient records with accuracy and confidentiality. • Coordinate communication between patients, families, and clinical staff. Required Qualifications: • Minimum of 3 years of experience in patient care coordination, medical office administration, or healthcare support strongly preferred. • Excellent communication and interpersonal skills, with the ability to support patients and providers. • Strong organizational skills and ability to manage multiple tasks efficiently. • Comfort with technology, including telehealth platforms, and willingness to guide patients through setup. • Ability to work independently in a fully remote environment.
