PATH Case Manager
George Junior Republic in Indiana seeks a community-based Case Manager for the new grant-funded Pre-Adjudicated and Truancy Help (PATH) program. This is a two-year program funded by the Indiana Criminal Justice Institute.Summary of the position: The PATH Case Manager is responsible for providing home-based and community-based quality services for youth and families involved with the PATH program. PATH Program Services:Home-based support and case managementCrisis ResponseGoal planning and mentoringDiagnostic assessments and individualized service plansTherapyPrimary Duties and Responsibilities:Compliance to organizational, program, and grant standardsProvide Client Care: This may include appearance at court hearings, connection with community resources, de-escalation, utilization of strength-based approaches.Meet productivity standards while adhering to program requirements. Effective verbal and written communication both internal and external to the organizationAbility to engage in individual and group supervision.Travel to various cities in order to support the needs of PATH participantsClients Served/Referral Sources:Youth ages 8-18 who may be referred for: Chronic absenteeismOppositional behaviorEarly substance useOther challenges linked to unmet behavioral health needsRequirements:Bachelor's or Master's degree from an accredited university.OrganizedExcellent written and verbal communication skillsMicrosoft experience Valid transportation, driver's license, and car insuranceGeorge Junior Republic offers competitive benefits:1.
