Job Title: Part-Time RN Nurse - Skilled Home Health Services

Overview:
Applesoft Homecare Services LLC is seeking a compassionate and highly skilled Registered Nurse (RN) to provide expert in-home healthcare services to our patients. This position involves delivering skilled nursing care, with a particular focus on wound care, PICC line care, and trach care. The role offers competitive per-visit rates, flexible scheduling, and mileage reimbursement.
Key Responsibilities:
Perform comprehensive assessments of patients' clinical conditions, focusing on wound care, PICC line care, and trach care.
Provide advanced wound care, including debridement, dressing changes, and infection prevention.
Manage and maintain PICC lines, including flushing, dressing changes, and monitoring for complications or infection.
Deliver trach care, including suctioning, monitoring for respiratory issues, and changing tubes as needed.
Develop and implement individualized care plans, in collaboration with the interdisciplinary team.
Educate patients and their families on wound care, trach care, and other medical procedures to promote self-care.
Monitor patient progress and make necessary adjustments to care plans, reporting significant changes to the supervising physician.
Maintain accurate and thorough documentation of patient care activities in compliance with Applesoft Homecare Services policies and healthcare regulations.
Ensure adherence to safety, infection control, and quality care standards.
Communicate regularly with healthcare team members to ensure a coordinated and holistic approach to patient care.
Provide compassionate, patient-centered care that promotes dignity and respect.
Qualifications:
Active RN license in the state of Texas.
Minimum of 2 years of clinical experience, preferably in home health or acute care, with specialized experience in wound care, PICC line care, and trach care.
Current CPR/BLS certification.
Valid drivers license and reliable transportation.
Strong assessment, critical thinking, and organizational skills.
Excellent verbal and written communication skills.
Compensation:
Competitive per-visit pay, commensurate with experience.
