This position provides skilled nursing care in a home care environment. The role involves assessing, planning, and documenting the homecare nursing process, as well as educating and providing instruction to patients and their families. The position also includes supervisory responsibilities, home health administrator responsibilities as assigned, and special project management as needed. If you have a contract in Ohio, you will have the flexibility to work remotely while taking on some tough challenges. As a skilled nursing professional, you will positively represent the company to both internal and external customers, always maintaining a professional and courteous manner. You will uphold the Optum core values in your work and adhere to company privacy guidelines, ensuring compliance with local, state, and federal regulations. You will provide services according to Optum policy and the prescriber's plan of treatment, individualizing patient assessments based on care/services being provided and patient needs. Your role will also involve ensuring that interventions respect and encourage the patient's ability to make choices, educating patients and families with a systematic approach, and initiating the plan of treatment/care pathway based on the analysis of information obtained. You will coordinate care with patient services centers, pharmacists, and other healthcare professionals to promote the continuum of care or service. Accurate communication of patient status information to the prescribing physician in a timely manner is essential, as is completing clinical record documentation per Optum policy. Participation in holiday and weekend duties as scheduled is required, and you must maintain a current CPR certification and comply with random drug testing. Routine travel is required, and you may be asked to participate in training or act as a preceptor. Additional duties may be assigned as necessary.

Requirements
• Current unrestricted nursing license (RN) for state of residence.
,
• 1+ years of obstetrical nursing experience.
,
• Proven ability to provide skilled nursing care for expectant mothers in the home care environment.
,
• Proven ability to modify the plan of care based on reassessment and changes in prescriber orders.
,
• Proven ability to continually assess the mothers' condition to ensure safety in the home environment.
,
• Proven ability to establish trust and build long-term relationships with expectant mothers.
,
• Proven ability to provide clinical support to key physicians/hospitals upon request.
,
• Proven ability to utilize Windows programs including Word, Email, and Internet access.
,
• Access to reliable transportation for travel to client and/or patient sites.

Nice-to-haves

• 3+ years of experience in labor and delivery or obstetrical nursing (high risk perinatal, antepartum)
