In this critical role, you will be instrumental in maintaining exceptional patient outcomes while promoting teamwork and collaboration among your colleagues. Your weekend responsibilities will involve scheduled administrative duties and workflow management from 8 AM to 5 PM.

Key Responsibilities
• Educate patients and caregivers regarding diagnoses, medications, infection control, and safety protocols in accordance with care plans.
• Provide administrative on-call support during evenings, weekends, and holiday rotations.
• Oversee weekend workflow, ensuring productivity and managing admissions effectively.
• Collaborate with interdisciplinary teams to enhance care coordination and improve patient outcomes.
• Ensure documentation aligns with state, local, and payer regulations, accurately reflecting care delivery.
• Supervise and support clinical staff working on weekends, ensuring adherence to established policies and procedures.
• Respond promptly to changes in patient conditions and communicate updates efficiently.
• Review documentation for accuracy and compliance with agency standards.
• Demonstrate proficiency in OASIS documentation and related reporting requirements.
• Manage the requisition of equipment and supplies to optimize patient care and autonomy.
• Uphold the confidentiality of patient information while honoring patient rights.

Required Skills
• A degree in Nursing from an accredited institution.
• An active RN license in the state of practice.
• Current CPR certification.
• Prior experience in home health care is preferred.
• Robust clinical skills paired with a professional and empathetic demeanor.
• Competence in utilizing computers and learning specific software relevant to the industry.
• Strong written, verbal, and interpersonal communication abilities.
