Collaborate with interdisciplinary teams to enhance care coordination and improve patient outcomes.
Ensure documentation aligns with state, local, and payer regulations, accurately reflecting care delivery.
Supervise and support clinical staff working on weekends, ensuring adherence to established policies and procedures.

Demonstrate proficiency in OASIS documentation and related reporting requirements.

Required Skills
A degree in Nursing from an accredited institution.
An active RN license in the state of practice.
Current CPR certification.
Prior experience in home health care is preferred.
