OHIO | Care Manager II, RN/LSW/LISW
About the position
The Care Manager II (RN/LSW) at AmeriHealth Caritas is responsible for providing support to complex members through care coordination and case management services. Responsibilities
Provide support to complex members for care coordination and case management services.

Engage with members, caregivers, and providers to assess and plan individual member goals.

Complete comprehensive person-centered assessments of physical, psycho-social, and environmental health.

Identify problems/barriers for care coordination and implement appropriate interventions.
,
Coordinate physical, behavioral health, and social services for members.
,
Manage medications, including regular reconciliation and adherence support.
,
Create and implement a plan of care to assist members in achieving health goals.
,
Identify short- and long-term goals with associated time frames for completion.
,
Document progress towards meeting goals and resolving issues.
,
Schedule follow-up calls and make appropriate referrals.
Requirements
Current and unrestricted Registered Nurse licensure in Ohio or licensed Social Worker (LSW or LISW) in Ohio.

Bachelor's Degree (or higher) in a health-related field preferred.
,
Minimum of 3 years of clinical practice experience with Medicaid populations.
,
Minimum of 3 years of case management experience preferred in behavioral health.
,
Proficiency in MS Office (Word, Excel, Outlook, Teams) and electronic medical record programs.
,
Valid driver's license, car insurance, and reliable vehicle.
