Assess, plan, and implement individualized treatment plans for patients in their homes.
Provide therapeutic interventions to help patients improve or regain daily living skills.
Collaborate with physicians, case managers, and other healthcare professionals to ensure coordinated, holistic patient care.
Educate patients and families on how to enhance independence and improve their quality of life through therapy techniques.
Document patient progress, treatment plans, and updates in accordance with regulatory standards.
Monitor patient outcomes and adjust treatment plans as necessary to achieve the best results.
Ensure that all therapy practices comply with state and federal regulations.
Qualifications:
Valid and unrestricted Occupational Therapist (OT) license.
Minimum of 2 years of experience as an Occupational Therapist, preferably in home health care.
Strong knowledge of home health therapy practices and patient care.
Excellent communication and interpersonal skills.
Ability to work independently while maintaining a collaborative approach with the healthcare team.
Detail-oriented with excellent time management and organizational skills.
Commitment to providing high-quality, compassionate care.
Experience with electronic health records (EHR) is a plus.
