Role Description This is a full-time, remote (work-from-home) position. The nurse must hold a current California license and will be responsible for independently managing their own chronic care management caseload. This role is entirely phone-based and focuses on patient outreach, care coordination, and ongoing support. Qualifications • Active RN or LPN license in the state of California (required) • Proven ability to work effectively in a remote environment • Experience conducting patient phone assessments and accurately updating medical records • Strong communication and collaboration skills for working with multidisciplinary care teams • Excellent organizational and time-management abilities to balance caseload demands • Proficient in Microsoft Office Suite (Outlook, Excel, Word) • A true self-starter who works independently, requires minimal supervision, and can problem-solve without frequent hand-holding Apply tot his job
