Nurse Practitioner - TeleHealth Located in Brooklyn, NY Salary: $130,000 - $150,000, including benefits; based on prior experience Qualifications: • Must have a current New York NP License (FNP or Adult NP preferred) • Experience in primary care or urgent care strongly preferred • Ability to conduct telehealth visits independently while on-site at the FQHC • Strong clinical assessment and prescribing skills • Strong written and verbal communication skills • Commitment to serving diverse and underserved patient populations Responsibilities: • Conduct virtual visits via video conference, phone calls, or messaging to assess, diagnose, and treat a wide range of conditions, from acute to chronic illnesses. • Develop and communicate treatment plans, prescribe medications, and order diagnostic tests. • Monitor patients' health remotely by analyzing data transmitted from home monitoring devices • Triage patient needs by phone or video, directing them to the appropriate level of care, and coordinating with other healthcare professionals for complex cases. • Educate patients on their health conditions, preventive measures, and how to use remote monitoring technology. • Maintain accurate and detailed electronic health records, documenting all patient interactions, histories, and test results. • Ensure video conferencing and other telehealth equipment are functional and assist patients with using the technology. • Navigate complex federal and state laws and regulations governing telehealth practice, licensing, and reimbursement.
