Focus Care Inc. is looking for experienced Nurse Practitioners to join our team in conducting in-home health assessments. This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement. As a Nurse Practitioner with Focus Care, you will: Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions. Provide education on medication management, disease processes, and preventive care. Identify and document diagnoses to support care management and active treatment. Collaborate with primary care providers to communicate findings and address potential gaps in patient care. Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required. Minimum Requirements: Completion of at least 30 in-home assessments per month. Valid Nurse Practitioner license in the state of Maine . Board Certification from ANCC or AANP Minimum of 1 year of clinical experience; experience with geriatric populations preferred. Strong communication skills and a commitment to delivering compassionate care. Reliable transportation for travel within the assigned areas. Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
