We serve all individuals regardless of their insurance status or ability to pay.Job DescriptionWe're seeking a Remote Full-Time Nurse Practitioner or Physician Assistant with Salud Family Health supporting our clinic in Sterling, CO.This position will work approximately 70% Telehealth and 30% Inbox (administrative work -management). There is no direct in-person patient contact associated with this job description. The Remote APP approaches his/her tasks in a team-based care fashion that in addition to clinic duties, also supports specific providers by helping address the daily accumulation of work within Salud’s EMR software system, eClinicalWorks (eCW). Salud will contribute 5% of your base salary each monthOur Mission is to provide a quality, integrated healthcare home to the communities we serveTelehealth patient care (70%) Conducts comprehensive assessment via Telehealth (video and/or audio), including history taking, review of systems, and evaluation of patient dataDiagnose and manage acute and chronic conditions within the APP scope of practice and established protocols and guidelinesDevelop, implement, and monitor individualized treatment plans, including prescribing medications, ordering and interpreting diagnostic tests, and providing patient education.Document patient encounters thoroughly and accurately in the Electronic Health Records (EHR) in a timely mannerProvide compassionate, patient-centered care through virtual channels, ensuring clear communication and rapport buildingIdentify patients requiring in-person care and coordinate appropriate follow-upHave exceptional written and verbal communication skills in order to deliver medical information clearly and emphaticallyParticipate in team meetings, case reviews, and clinical quality improvement initiativesWorks in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that the patients obtain the highest standards of integrated healthcareAbility to recognize limitations and seek help when necessaryKeeps abreast of current evidence-based clinical guidelines, standards of care, and standards of practice in the provision of health center servicesPerforms other duties as assignedBubble’/Inbox Management (30%) Efficiently and proactively manage the administrative workload generated with eCW’s ‘bubbles’ assigned to themselves and for specific physicians and APPs who are either out on leave or have left Salud without a replacement being yet hired. The specific physicians and APPs will be identified to the Remote APP by the Chief Medical Officer.Triage and respond to a wide range of patient messages, including but not limited to:Symptom enquiry and advice requestsMedication refill requestsLab/test result communication, interpretation, and follow-up planPost-visit follow-up questionsReferral follow-up and questionsDocument review and signatureExercise independent clinical judgement within established protocols to:Address patient concernsDetermine the need for a Telehealth visit, in-person visit, or other interventionRenew prescriptions based on set protocolsEnsure timely response as established by protocolsMaintain meticulous documentation of all inbox interactions, decisions made, and actions within the EHRWorks in conjunction with other Medical Providers, Dental Providers, Behavioral Health Providers, and all other staff to ensure that patients receive the highest standards of integrated healthcareIdentify opportunities for process improvementAbility to work independently, recognize limitations, and seek help when necessary or know when to escalate appropriatelyKeeps abreast of current evidence-based clinical guidelines, standards of care, and standards of practice in the provision of health center servicesPerforms other duties as assignedQualificationsEducation:Completion of a certified Physician Assistant program with state and national certification as evidenced by a certificate of Physician Assistant status by the National Commission on Certification of Physician Assistants (NCCPA) and by the State Board of Medical ExaminersorCompletion of an accredited Nurse Practitioner/Certified Advanced Practice Registered Nurse program with state and national certificationLicensure:Possess a Physician Assistant license or an Advanced Practice Registered Nurse license in good standing in the State of ColoradoExperience:2 to 5 years’ experience as a Nurse Practitioner or Physician Assistant in primary care, internal medicine, or family medicine preferredExperience with Telehealth platforms and delivering virtual careKnowledge, Skills, and Abilities:Good physical health and emotional stability are required to maintain pace and adequate interpersonal relationshipsAbility to relate with co-workers, patients, and their sensitivity to each individual's needsMust work well with other patient care team membersConcern for the quality of medical care and satisfaction of patientsAbility to make appropriate interpersonal relationshipsHighly motivated, tech-savvy, and detail-oriented for this unique position.
