Nurse Practitioner Job Description
Job Summary
We are seeking a PARTNERSHIP with a dedicated and skilled Nurse Practitioner or Physician Assistant for a unique opportunity. The ideal candidate will be responsible for overseeing an operation that provides high-quality patient care via telephone or video call. Patients and staff will be provided. The Nurse Practitioner will work collaboratively with nurses and medical assistants and review the notes our staff will provide. The NP/PA will file the proper CPT codes.
Duties
• Ability to bill Medicaid and Medicare.
• Conduct thorough patient assessments.
• Assign health regimens.
• Develop and implement individualized treatment plans based on patient needs.
• Perform diagnostic tests and interpret results to guide treatment decisions.
• Manage acute and chronic health conditions, including psychopharmacology for mental health patients.
• Deliver memory care services to patients with cognitive impairments.
• Educate patients and families about health management, disease prevention, and wellness strategies.
• Maintain accurate patient records and documentation in compliance with healthcare regulations.
Qualifications
• Current Nurse Practitioner/ PA certification and state license.
• Ability to bill Medicare and Medicaid
• Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
