The Nurse Patient Navigator serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Mount Sinai South Nassau and in the community. Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians.QualificationsResponsibilities:Education Required
Must complete in-house orientation program
Graduate of an accredited program for Registered Professional Nursing. (Baccalaureate Degree in nursing required ).
Must complete annual mandatory inservice education (Hospital/Nursing).
Experience And Skills
Excellent interpersonal skills
Speaks, reads and writes English
Clinical experience/skills relevant to the assigned area of practice.
Computer proficiency.
License/Certifications
Certifications for assigned area of practice: Navigator Certified within one (1)year of hire.
Current and valid New York State registration authorizing practice as a Registered Professional Nurse (RN)
Certifications required for assigned area of practice:
o BLS (all nurses).
o Certification related to specialty area preferredWorking ConditionsOther information:Working Conditions
Personal Protective Equipment (PPE) worn
Frequent, prolonged standing/walking.
Lifts, positions and/or transfers patients/equipment.
Protective clothing/equipment worn (face shields, gloves, impermeable gown, particulate respirator).
