You will collaborate closely with other members of the healthcare team to ensure that our patients receive the best possible care.Key Responsibilities:Conduct initial assessments of patients' physical and emotional condition to develop a comprehensive care plan.Provide skilled nursing care, including wound care, medication management, and symptom management.Educate and communicate effectively with patients and their families about their condition, treatment plans, and self-care techniques.Collaborate with other members of the healthcare team, including physicians, social workers, and therapists, to develop and implement the patient's care plan.Monitor and evaluate patients' progress and make necessary adjustments to the care plan.Ensure all documentation is accurate, complete, and in compliance with state and federal regulations.Provide emotional support and comfort to patients and their families during the end-of-life journey. Requirements:Associate’s DegreeValid Maine RN License (or eligible)BLS CertificationCurrent driver’s license with reliable transportation and agency required auto liability insuranceCertified Nursing Assistant/Home Health Aide: Responsible for providing personal care and may at times provide meal preparation or light housekeeping to a designated group of patients as directed by a Registered Professional Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist. Provides routine individualized healthcare such as changing bandages and dressing wounds, applying topical medications to the elderly, convalescents, or persons with disabilities at the patient's home or in a care facility. Assuming appropriate training, may also perform lab work, and other patient care related tasks.Key Responsibilities:Maintains current knowledge of organizations policies and procedures. Adheres to governmental and industry specific regulations/laws. Maintains appropriate related documentation.Provides excellent service with focus on patient and/or customer experience and ensures all functions related to customer service supports the department, member organization, and patient related goals.Prepares, maintains, and distributes documents or information.Follow established procedures and standards to limit the spread of infection.Attends and participates in meetings.Works to meet patient's psychological, emotional, or physical needs.Attends education sessions, workshops, and other continuing education opportunities to enhance or gain new skills.Meets established productivity benchmarks.Works to ensure a safe environment for staff and patients.Works with members of the team or department in an effective and efficient manner.Shares knowledge and serves as resource in a team environment.Leverages software and/or hardware relative to job function.Other Duties as assigned.Requirements:Valid Maine CNA License (or eligible)BLS CertificationCurrent driver’s license with reliable transportation and agency required auto liability insurancePhysical Therapist:Physical Therapists:he Physical Therapist will handle evaluating and treating patients with physical disabilities, injuries, or chronic conditions in their homes. The ideal candidate will have experience in home care and hospice settings, as well as a strong commitment to providing exceptional patient care.Key Responsibilities:Conduct initial evaluations of patients to assess their physical function, mobility, and pain levels.Develop individualized treatment plans based on patients' needs and goals.Provide hands-on therapy, exercises, and other techniques to improve patients' strength, range of motion, balance, and coordination.Educate patients and their families on home exercise programs and proper body mechanics.Monitor patients' progress and adjust treatment plans as needed.Communicate with other members of the Home Care & Hospice team, including nurses, social workers, and doctors, to coordinate patient care.Document all patient evaluations, treatments, and progress in a prompt and correct manner.Participate in ongoing education and training to stay updated on the latest physical therapy techniques and advancements.Adhere to all company policies, procedures, and regulations to ensure quality patient care and safety.Requirements: Bachelor’s DegreeValid Maine Physical Therapy License (or eligible)BLS CertificationCurrent driver’s license with reliable transportation and agency required auto liability insuranceNorthern Light Home Care & Hospice is providing direct, personalized care throughout Maine - from South Portland to Fort Kent with our roots dating back to 1912.
