Job Title: Networker / Community Outreach Representative (Commission-Based)Company: It’s Your Home Care Location: Harrisburg, PA & Surrounding Counties (Field-Based) Employment Type: Commission-Based (Independent Contractor or 1099) Reports To: Director of Operations Position Summary It’s Your Home Care is seeking an energetic and motivated Networker / Community Outreach Representative to help grow our agency’s consumer base across Central Pennsylvania. This role is fully commission-based, offering unlimited earning potential (from $0 up to $150,000 per year) based on performance. The Networker will focus on building and maintaining relationships with Service Coordinators, Case Managers, Hospitals, and Community Programs within Pennsylvania’s OLTL, CHC, OBRA, and Act 150 waiver systems. The goal is to generate new referrals and increase consumer enrollments for home care services. Key Responsibilities • Develop and maintain strong referral relationships with case managers, support coordinators, and community agencies. • Visit hospitals, rehabilitation centers, independent living facilities, and local community organizations to introduce our agency and services. • Present and explain the benefits of It’s Your Home Care to prospective consumers and families. • Identify referral opportunities within managed care organizations (UPMC, AmeriHealth Caritas, PA Health & Wellness, Keystone First). • Attend networking events, community fairs, and health expos to promote the agency. • Coordinate with the intake team to ensure referrals are properly logged and followed up on. • Maintain weekly activity reports and outreach logs to track referral progress and conversion. Qualifications • Proven experience in marketing, sales, or outreach within the home care, healthcare, or human services industry. • Strong existing relationships with Service Coordinators and MCO contacts are a major plus. • Self-motivated, professional, and results-driven. • Excellent interpersonal and communication skills (bilingual preferred). • Ability to work independently, manage your own schedule, and meet performance goals. • Reliable transportation and willingness to travel throughout Central Pennsylvania (mileage reimbursement available when applicable). Compensation • Commission-based compensation only. • Earning potential ranges from $0 to $150,000+ per year, depending on performance. • Commission structure based on successful new consumer enrollments (details discussed during interview). • Flexible schedule — you control your own hours and workload.
