Job Summary
Cancer Support Community (CSC) is seeking a dedicated, professional, and passionate part-time Navigator, Helpline. The Cancer Support Helpline (CSH) offers personalized, toll-free phone support services to anyone whose life has been impacted by cancer.
The Helpline is open Monday-Thursday 11 AM to 8 PM ET and Friday 11AM ā 6 PM ET. CSH Navigators will work as needed during the daytime shifts of 11AM to 5PM ET. Applicants must be willing to work some evenings until 8 PM ET. Candidates will agree to work between 20 and 29 hours per week in this role.
Essential Functions
ā¢ Address barriers by providing individual assistance to patients, survivors, and families.
ā¢ Provide services to patients and caregivers via telephone and chat following the CSC Proactive Navigation model based on the Expanded Chronic Care Model (ECCM) and utilize a validated distress screening tool for evaluation/screening and psychosocial assessments.
ā¢ Services include referral to local, regional, and national resources; emotional support, distress screening through CSCās CancerSupportSourceā¢ (CSC); and Open to Optionsā¢ decision support by phone. Community Navigators refer callers to CSC network partners when applicable.
ā¢ Provide interventions to callers including care coordination, advocacy, education, emotional support, and linkage to resources.
ā¢ Proactive follow-ups with callers, recording of information, measuring outcomes of interventions.
ā¢ Assess callers in crisis, work to ensure safety and connect them to the proper level of care and resources for the situation at hand.
Skills And Qualifications
Performs other duties as required by management
ā¢ Attention to detail and ability to keep accurate and up-to-date records of callers and their interactions, including outcomes and any actions taken.
ā¢ Flexibility and adaptability to changes in workflows, technology and available resources.
ā¢ Strong active listening skills to identify caller needs and provide referrals to local, regional and national resources, including housing, lodging, transportation, oncology treatment centers, clinical trial education, etc., based on those specific needs.
ā¢ Triage acumen to appropriately escalate callers to specialists in financial, clinical trial, pediatric oncology and genetics/genomics, or to implement crisis management protocol when necessary.
ā¢ Bilingual speaker a plus
EDUCATION: Masterās degree in social work
Experience
At least two (2) years of hospital based, non-profit or social services-based oncology patient navigation and/or social work. Experience in providing education to oncology populations.
Required Certificates/Registrations
Social Work license in good standing from the state in which the candidate resides.
Oncology social work certification is preferred, not required. Navigation certification is preferred not required.
WORK LOCATION
CSC has offices located in Washington D.C. and offers in-person, hybrid, or fully remote work for this position.
Salary And Benefits
Pay is $40/hour. In addition to a competitive pay rate, we offer a supportive work-from-home culture, paid sick time, a 403(b) retirement plan, and cell phone and internet reimbursement to support your remote work needs.
Dependent on experience. In addition to a competitive base salary, we offer statutory benefits required by law.
