MLTC Social Work Care Manager/ Remote

Job Description Established and growing MLTC health plan designed to help keep people independent and living life on their own terms is currently seeking a Social Work Care Manager (MSW, LMSW or LCSW). MLTC Care Manager - REMOTE Job will be fully remote after the training period of 2-4 months (working 2 days per week in the Manhattan NY office and 3 days remote training). $80,000 to $90,000+ with Generous Benefits $15,000 sign on bonus for candidate with NY State LMSW or LCSW Bilingual candidates strongly encouraged to apply! Training Location: Manhattan, NY Full-time, Permanent Role - Monday through Friday, Business Hours This position will provide case management to our members including medical, social, psychological, physical and spiritual needs by Developing, implementing and monitoring the care plan, in conjunction with the PCP, caregivers and other team members, to help meet the member's needs. Requirements: • 3-5 years clinical social experience in long term care, managed care, home care, geriatrics or hospice • Prior experience in an interdisciplinary service delivery environment preferred. • Working knowledge of Medicare and Medicaid regulations • Prior experience in an interdisciplinary service delivery environment.
