Company :Allegheny Health NetworkJob Description : GENERAL OVERVIEW:Maintains safety and structure of the milieu, observing and recording patient information pertinent to the treatment plan and reports information to appropriate staff.ESSENTIAL RESPONSIBILITIES:Collaborates with the multi-disciplinary team to provide for an on-going assessment of changes in patient status and gathers pertinent patient care data using all available resources; provides crisis intervention to maintain safety (40%)Provides direct patient care and activities, such as activities of daily living, vital signs, specimen collection, assistance with exams, treatments and procedures and patient focused testing; group facilitation, reports any changes in physical, mental, or emotional conditions to nursing staff and documents same; suggests modifications to the master treatment plan (40%)Leads or co-leads in social, recreational, and therapeutic activities that enhance patient's social relationships and interpersonal skills (15%)Participates in unit activities such as supplies stocking, patient transport, and equipment / appliance cleaning, checks and retrieval.
