About the position
Novant Health is seeking a Medication Management Specialist, to join our ambulatory pharmacy team, to deliver optimal medication management across all dimensions of care, every time. The role involves partnership with NHMG providers and clinical staff to provide remarkable patient care, assisting with Medication Therapy Management and other clinical pharmacy outreach programs, and addressing insurance, reimbursement, and payment issues as appropriate by communicating with insurance providers, patients, and pharmacists.
Responsibilities

Assists with Medication Therapy Management and other clinical pharmacy outreach programs
,
Assists pharmacists with reviewing and processing refills, prior authorizations, and patient assistance applications
,
Obtain medication histories, including fill history, for medically complex patients
,
Addresses insurance, reimbursement, and payment issues as appropriate by communicating with insurance providers, patients, and pharmacists
,
Monitors adherence by contacting the patient at least monthly to ensure adherence and determine refill needs as applicable
,
Contacts patients and schedules appointments as necessary
,
Contributes to process improvement projects
,
Ability to communicate effectively both orally and in writing while having a collaborative approach to patient care
,
Demonstrating proficiency with equipment and software
Requirements
5+ years of outpatient pharmacy or managed care experience
,
High School Diploma or GED
,
PTCB Certification and NCBOP License
,
Considerable knowledge of medication names (both trade and generic) and their doses, routes of administration and adverse effects
,
Demonstrates patient interview skills and strong written and verbal communication skills with other healthcare professionals
,
Ability to follow verbal and written instructions
,
Basic technology skills

Highly motivated individual who is self-directed and able to execute tasks in a quickly changing environment

Ability to multi-task, prioritize essential tasks and meet deadlines
