Key Responsibilities

Maintain accurate and up-to-date patient records, adhering to healthcare regulations and privacy standards.
Collaborate with healthcare providers to tailor care plans to meet each patient's unique needs.
Qualifications
Experience in chronic care management or remote patient monitoring is a plus.
Exceptional organizational and multitasking abilities.
Strong communication and interpersonal skills.
Familiarity with electronic health records (EHR) systems is preferred.
