Key Responsibilities:
Engage with patients to understand their care needs and address healthcare concerns
Collaborate with medical practice staff and Case Management teams to deliver comprehensive care
Accurately document patients' medical history and issues in electronic health records (EHRs)
Meet assigned Key Performance Indicators to drive patient outcome improvements
Utilize various communication methods to effectively engage high-risk patients
Requirements:
Certified or Registered Medical Assistant or certified EMT
Proficiency in medical terminology and primary care physician practices
Experience with electronic health records (EHRs) and care management analytics
Exceptional written, visual, and verbal communication skills
Ability to work independently as part of a patient's care team under supervision
Preferred Qualifications:
Familiarity with EHR systems such as EPIC, Athena, or Cerner
Experience in motivational interviewing and behavioral change management
Strong critical listening skills and empathy in patient coordination
Ability to manage multiple tasks while maintaining attention to detail
Proven success in a remote work environment
