Responsibilities
Ā Assess, plan, and implement individualized care strategies for patients.

Ā Identify and initiate referrals for social service programs, including financial and community services.

Ā Manage the care plan throughout the continuum of care as a single point of contact.

Ā Communicate health-related information to stakeholders to ensure quality coordinated care.

Ā Advocate for patients and families to ensure their needs and choices are represented.
Requirements
Ā High School Diploma/GED (or higher).

Ā Current, unrestricted LPN/LVN license in the state of Texas.
,
Ā 3+ years of clinical experience in a health care role.
,
Ā Intermediate level of Microsoft Office proficiency, including Word, Excel, and Outlook.
,
Ā Ability to travel up to 10% of the time for team meetings and member engagement.
Nice-to-haves
Ā Certified Case Manager (CCM).

Ā Experience with Texas STAR and CHIP case management.
,
Ā Experience working with members with serious mental illness (SMI).

Ā Experience in managed care.
,
Ā Bilingual in English/Spanish or Chinese/Mandarin.
