Responsibilities:
• Monitor patient vitals and health indicators remotely, using state-of-the-art software and tools.
• Manage patients with chronic illnesses by providing telephonic coaching, guidance, and health education.
• Develop personalized care plans based on individual patient needs.
• Communicate regularly with patients, care teams, and physicians to ensure optimal patient outcomes.
• Document and maintain accurate, comprehensive patient records.
• Enroll patients in virtual care services.

Requirements & Qualifications:

License: Licensed Practical Nurse (LPN)

Experience:
• Minimum of 2 years of nursing experience, with a focus on chronic disease management.
• Technical Skills: Proficiency in using medical software and digital health tools.
• Communication Skills: Excellent communication, interpersonal, and patient management skills.
• Independence: Ability to work independently and handle multiple tasks simultaneously in a remote environment.
