LPN / Telehealth Nursing / Shift Varies / Rotating Weekends
About the position Responsibilities • Demonstrate strong competency in using various telecommunication technologies to provide direct virtual nursing care. • Triage and prioritize patient needs, collaborating with patients to develop care plans. • Assist patients with prescription management across specialties. • Complete prior authorizations as needed and route medication renewal orders to providers for review. • Utilize current evidence-based nursing practices and guidelines. • Document, update, and maintain patient medical records. • Act as a liaison between patient and provider. • Educate patients on the provider's plan of care. • Train, orient, and educate new team members and students. • Participate in staff meetings and organizational meetings. Requirements • High School Diploma or GED required. • Graduate of an accredited Licensed Practical Nursing Program. • Valid, active, unencumbered nursing license or temporary permit approved by the Georgia Licensing Board. • Current BLS certification required. • Strong telephone communication skills and computer skills. • Effective communication skills over various mediums. • Knowledge of electronic medical records and e-prescribing. • Proficiency with Microsoft Word, Excel, PowerPoint, TEAMS, Outlook and eMR. Nice-to-haves • Experience in telehealth nursing. • Familiarity with the Synergy model for Care. • Ability to manage multiple tasks in a fast-paced environment.
