LPN - Remote Chronic Care Manager
Job Description: • We are seeking a compassionate and patient-centered Licensed Practical Nurse (LPN) to join our care team as a Chronic Care Manager. • This fully remote position plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education. • Ideal candidates are highly organized, strong communicators, and committed to making a difference in patients’ day-to-day lives. • Key Responsibilities include conducting comprehensive patient assessments, collaborating with interdisciplinary care teams, monitoring patient health status, educating patients and families, and maintaining accurate documentation.
