LPN Clinical Coordinator - Remote Opportunity in Marion County, IN

Join United Healthcare in simplifying the healthcare experience, creating healthier communities, and removing barriers to quality care. As a Care Coordinator, you will play a vital role in managing complex members and making a difference in the lives of millions of people.
We are seeking a compassionate and experienced LPN to join our care management team, working remotely from Marion County or surrounding areas. As a primary care manager, you will be responsible for assessing the medical, behavioral, and socioeconomic needs of older adult members and developing individualized care plans to promote improved health outcomes.
Key Responsibilities:

Conduct comprehensive needs assessments and develop person-centered care plans
Engage members face-to-face and/or telephonically to provide support and guidance
Partner with internal care teams, providers, and community resources to implement care plans
Provide referrals and linkages to community-based services and resources

Requirements:

LPN license with 3+ years of clinical experience working with older adults
1+ year of experience with local behavioral health providers and community support organizations
1+ year of experience with MS Office, including Word, Excel, and Outlook
Resident of Indiana with reliable transportation and ability to travel within assigned territory

Preferred Qualifications:

2+ years of experience in long-term support services or working with older adults
Experience with Motivational Interviewing, Stages of Change, Trauma-Informed Care, and Person-Centered Care
Background in Managed Care and experience working in team-based care
Bilingual in Spanish or other language specific to market populations
