LPN Care Coordinator | Inbound, Remote

We’re looking for a Licensed Practical Nurse (LPN) to join our Inbound Care team. This fully remote position offers a meaningful way to use your clinical expertise while maintaining a strong work-life balance. Responsibilities • Answer inbound clinical calls and patient inquiries (phone, email, chat) • Make outbound follow-up calls as needed • Assist in triaging patient concerns and escalating appropriately • Review and transcribe medical records • Resolve urgent patient account or care issues • Coordinate access to additional patient resources • Obtain/send medical records and documentation via fax • Collaborate with Triage Nurses, Provider Relations, and Pharmacy Techs • Maintain high performance across key metrics including: • Call Answer Rate • After-Call Work Time • Quality Scores • Email/Chat Response Time • Uphold scheduled work hours and consistent attendance Requirements • Active, compact LPN or LVN license • Graduation from an accredited nursing program (LPN/LVN) • Experience following clinical protocols and triaging patient concerns • Strong communication skills and ability to multitask • Reliable internet connection and dedicated work environment • Team-oriented with a commitment to high-quality patient care Benefits • 100% remote work • Full-time schedule with available day, bridge, or night shifts • Equipment provided • Health, dental, and vision insurance • Paid time off and holidays • Supportive and collaborative team culture • Emphasis on work-life balance Work Environment This is a fully remote position.
