Key Responsibilities
Conduct diagnostic evaluations, diagnoses, psychotherapy, treatment planning, and consulting services, both in-person at Skilled Nursing Facilities and remotely using our Telehealth software.
Complete documentation efficiently in our electronic medical record system (Athena I) from the comfort of your home or office.
Adhere to agency policies and procedures to ensure the highest standard of care.

Requirements
Master's degree in Social Work, Counseling, or a related field, with Arizona licensure as an LCSW, LMSW, LMFT, LAMFT, LPC, or LAC.
National Provider Identifier (NPI).
Professional Liability Insurance.
Credentialing with Medicaid/Medicare is a plus!
Some facilities may require up-to-date vaccinations or appropriate exemptions.
Tuberculosis (TB) Test.
CPR/AED/First Aid Certification.
Skills
Basic computer proficiency.
Excellent written communication abilities.
Attention to detail and strong organizational skills.
Strong interpersonal skills and ability to work under pressure.
Fluency in Spanish is a plus!
