Licensed Telehealth Physical Therapist (Florida Home Health Care) - Contract to Hire
Location: Remote (Must be licensed to practice in Florida) Job Description We are a growing home health care agency in Florida seeking a licensed Physical Therapist with telehealth experience to perform patient evaluations via secure video sessions. The ideal candidate will be professional, compassionate, and experienced in working with patients in a home health setting. Responsibilities Conduct telehealth physical therapy evaluations for new patients referred to our home health agency. Assess patient mobility, strength, and functional needs through secure video conferencing. Develop individualized care plans and recommendations for in-home therapy services. Collaborate with our nursing staff and care coordinators to ensure patient care goals are met. Complete required evaluation documentation accurately and in a timely manner. Requirements Current Physical Therapy license in the state of Florida (required). Prior experience providing telehealth evaluations. Strong communication and interpersonal skills. Reliable internet connection and ability to use telehealth platforms. Experience working with home health care patients is a plus. Position Details Remote / telehealth position.
