Job Summary
We are seeking a licensed practical nurse with a passion for delivering exceptional patient care in a home health setting. As a Home Health LPN, you will work under the direction of a Registered Nurse (RN) to provide skilled nursing interventions, teaching, and training activities to patients with various illnesses, rehabilitative needs, and preventative care. Key Responsibilities:
Provide professional nursing services under the direction of a RN in compliance with the state's Nurse Practice Act, licensure/certification requirements, healthcare standards, and CenterWell Home Health policies and procedures.
Conduct patient assessments, develop and implement individualized plans of care, and evaluate patient outcomes.
Provide skilled nursing interventions, teaching, and training activities to patients and their families.
Utilize a holistic approach in the provision of problem-specific therapeutic interventions.
Confer with the RN Supervisor regarding needed changes in the Plan of Treatment.
Accept verbal orders from physicians, as permitted by state law/regulations/Nurse Practice Act, and communicate these orders to the RN Supervisor.
Maintain contact with patients, physicians, clinical managers, and other members of the healthcare team regarding patient/family needs and status changes.
Participate in care coordination activities and discharge planning as appropriate.
Maintain the highest standards of professional conduct in relation to confidential information.
Requirements:
Essential Qualifications:
Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
Current nursing license in the practicing state.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
Preferred Qualifications:
Experience with electronic medical records (EMRs) and home health software.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
