Licensed Practical Nurse - Home Health Full Time
Job Description
A Licensed Practical nurse performs nursing functions under the direction of a registered nurse, follows the plans of care, participates in care planning, and performs nursing actions in accord with company policy and state law.
Responsibilities
ā¢ Documents observations and care given in accord with company process according to company policies and procedures. Administers medications.
ā¢ Administers basic nursing care measure to the rehab patient and assists in giving care to the acutely ill rehab patient relative to patient age, developmental needs, and understanding.
ā¢ Admits, transfers, and discharges patients. Observes, records, and reports subjective patient symptoms.
ā¢ Assists the RN in giving physical and emotional support to the patient.
ā¢ Provides for dietary and fluid needs of the patient such as nourishment, feeding, tube feeding, observing and recording IV fluids and maintaining an accurate fluid intake and output.
ā¢ Takes and record vital signs: temperature, pulse, respirations, and blood pressure.
ā¢ Adheres to policies and procedures specific to patient rights.
ā¢ Maintains a clean and safe environment; identifies and reports hazards.
ā¢ Meets all requirements for mandatory in-services.
ā¢ Adheres to Brooks Rehabilitation and Home Health Division policies/procedures specific to universal precautions when delivering patient care.
ā¢ Participates in On Call Rotation
Qualifications
ā¢ Licensed as an LPN in the state of Florida.
ā¢ 1 year of experience.
ā¢ IV Certified.
ā¢ Current hands on BLS certification.
