Job Overview We're seeking an experienced and skilled LPN to join our home health team in Buford, GA. As a key member of our care team, you'll play a vital role in delivering high-quality patient care and supporting our patients' overall well-being. This is a 100% remote position, offering you the flexibility to work from the comfort of your own environment. Key Responsibilities Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care Follow the nursing care plan for each patient and accurately report and record their condition and care Prepare clinical/progress notes and summaries to ensure comprehensive documentation Assist patients with activities of daily living while teaching them appropriate self-care techniques Maintain a safe environment for patients and follow universal precautions when providing care Assist physicians and Registered Nurses in complex nursing situations Follow emergency procedures in the event of a catastrophic event or significant change in a patient's condition Essential Qualifications To be considered for this role, you must meet the following essential qualifications: Current state license as an LPN Graduate of an accredited program of practical nursing At least 1 year of experience as an LPN in a home care setting Tracheotomy experience required Proof of current CPR certification Valid driver's license and reliable transportation Ability to work a flexible schedule and travel locally Preferred Qualifications While not mandatory, the following qualifications are highly desirable: Excellent interpersonal skills and ability to communicate effectively with patients, families, and healthcare teams Strong clinical skills and ability to work independently Experience with electronic medical records and other healthcare technology Skills and Competencies To succeed in this role, you'll need to possess the following skills and competencies: Strong clinical judgment and decision-making skills Ability to work effectively in a remote environment Excellent communication and interpersonal skills Ability to prioritize tasks and manage time efficiently Compassionate and patient-centered approach to care Career Growth Opportunities and Learning Benefits At Beacon Care, we're committed to supporting the ongoing growth and development of our team members.
