Licensed Practical Nurse – Chronic Care Management (Remote)
Job Summary:
SAS Health Tech is looking for a Licensed Practical Nurse (LPN) to join our growing remote Chronic Care Management team. In this role, you will play a key part in supporting patient care through remote telehealth services.
Key Responsibilities:
• Monitor and review patient symptoms and medications.
• Address patient inquiries about their health during scheduled telehealth sessions.
• Collaborate with facility CNAs to arrange remote telehealth visits.
• Create detailed 20-minute patient entries and update records in our EHR system.
• Develop and implement individualized care plans based on established protocols.
• Stay informed about patient care policies, client rights, and evolving healthcare guidelines.
• Recognize signs of improvement or decline in various medical conditions.
