We specialize in Transitional Care Management (TCM), Chronic Care Management (CCM), and Remote Patient Monitoring (RPM) services. Position Summary We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to provide telehealth-based chronic care management and care coordination. The LPN will support patients with chronic conditions, help manage care plans, and ensure continuity of care through regular follow-up and education. Responsibilities • Conduct monthly and ongoing Chronic Care Management (CCM) calls • Provide patient education and support in managing chronic conditions • Update care plans under NP or physician supervision • Coordinate care and communication among providers • Document all patient interactions in the EMR (AthenaHealth) • Identify potential complications and escalate as needed Qualifications • Active LPN license in Florida and/or New York (required) • Minimum 2 years of clinical experience (home health, primary care, or telehealth preferred) • Strong communication, clinical judgment, and documentation skills • Reliable internet and home workspace • Familiarity with EMR systems (AthenaHealth preferred) • Preferred billingual Benefits • Remote work-from-home flexibility • Competitive hourly pay + performance incentives • Supportive, collaborative team culture • Paid training and professional development • Opportunity for advancement within VirtuMedex Schedule: • Flexible scheduling available (part-time or full-time hours) Work Location: Remote Job Types: Full-time, Part-time Pay: $30.89 - $37.21 per hour Expected hours: 5 – 40 per week Benefits: • 401(k) • 401(k) matching • Flexible schedule • Paid time off Language: • English and Spanish?
