About the position

The LTSS Service Coordinator is responsible for conducting service coordination functions for a defined caseload of individuals in specialized programs. This role involves facilitating the Person Centered Planning process, conducting assessments, developing comprehensive support plans, and managing the physical and behavioral health needs of individuals. The coordinator acts as a single point of contact for members, ensuring effective care coordination and support.

Responsibilities
• Conduct face-to-face program assessments using various tools and predefined questions.

• Apply motivational interviewing techniques for evaluations and management of individual needs.
,
• Coordinate cases with clinical healthcare management and interdisciplinary teams.
,
• Document short and long-term service and support goals in collaboration with the member's care team.
,
• Identify members that would benefit from alternative levels of service or other waiver programs.
,
• Mentor and assist in the training of new staff members.
,
• Submit utilization/authorization requests to utilization management with supporting documentation.
,
• Report critical incidents to appropriate internal and external parties.
,
• Assist in appeals, member grievances, and state audits.

Requirements
• Bachelor's degree in Social Work or a related field.
,
• Minimum of 2 years of experience working with a social work agency.
,
• CPR Certification.
,
• Certified Case Manager certification preferred.
