Position: Home Health Registered Nurse (PRN)

Compensation: Competitive salary ($33.00 to $39.00 per hour) and comprehensive benefits package
About the Role:
As a compassionate and skilled Home Health Registered Nurse (PRN), you will have the opportunity to make a meaningful impact on the lives of patients and their families. You will provide high-quality, patient-centered care in the comfort of patients' homes, working collaboratively with our team of healthcare professionals to develop and implement personalized care plans.

Responsibilities:
Coordinates and oversees all aspects of patients' home care, including developing and implementing care plans
Collaborates with patient care teams to provide high-quality, patient-centered care
Educates families on patient condition, needs, and care
Performs physical assessments, checks vital signs, and manages medications
Administers injections, IV therapy, or tube feedings, and maintains catheters
Completes documentation timely and accurately, and communicates patient progress to the clinical manager, physician, and care team
Supervises LPNs and HHAs, and participates in quality improvement initiatives
Qualifications:
Current RN license, specific to the state(s) you are assigned to work
Minimum one year of clinical experience as a registered nurse (RN), or completion of an RN Intern program
Minimum six months of home health experience
Current CPR certification
Valid driver's license, reliable transportation, and liability insurance
Able to assess patient status and identify requirements relative to age-specific needs
Proficient in the use of computers, including navigation of computer desktop, email, and online HR services
