Responsibilities
Assist patients during admissions, transfers, and discharges, ensuring their personal care needs are met in accordance with their wishes and care plans.
Review care plans daily to determine if changes have been made to the patient's daily care routine.
Qualifications
CNA license certification prior to hiring.
Current CPR and fingerprint clearance card.
Minimum of six (6) months CNA/LNA experience preferred, with recent Long-Term Care/Skilled Nursing Facility or Transitional Rehab experience.
