Job Summary:
We're seeking a highly motivated and compassionate individual to join our team as a Community Care Worker. In this role, you'll work with clients to achieve program goals, utilizing our proprietary platform and data management system. Your responsibilities will include:
Providing population health services to assigned clients on an ongoing and emergency basis
Developing and maintaining accurate knowledge of program requirements, tools, and community resources
Organizing and documenting daily schedules, adhering to safety protocols, and ensuring compliance with HIPAA and confidentiality policies
Demonstrating competency by meeting or exceeding minimum caseload requirements and department work performance expectations
Communicating effectively with supervisors, customers, and co-workers to build rapport and resolve issues
Requirements:
To succeed in this role, you'll need:
An Associate's Degree in social work or a related field, or a High School Diploma/GED with 4 years of related experience and/or training in healthcare and/or customer service
2 years of experience and/or additional education in social work or a related field, with knowledge of medical terminology and/or experience in a customer service position (preferably in a healthcare setting)
A valid driver's license, current automobile insurance, and availability to work evening hours (5:00 pm - 8:00 pm) and some weekends
