In this fully virtual role, you’ll monitor patient alerts, conduct wellness outreach, support care coordination, and promote patient engagement from the comfort of your home.In this impactful role, you’ll: Support patients with real-time health monitoringRespond to alerts and deliver timely, life-improving interventionsEducate and empower patients through ongoing careCollaborate with clinical teams to enhance outcomes Respond to alerts and deliver timely, life-improving interventionsIf you're ready to bring your clinical expertise and compassion to the forefront—from the comfort of your home—we want to hear from you!Start Date: October 20th, 2025Compensation & IncentivesCompetitive base hourly rateTeam members are eligible for additional compensation based on the timing of hours worked, as follows:Evening Differential: An additional $1.50 per hour will be paid for all hours worked after 7:00 PM Eastern Time.Late-Night Differential: An additional $2.00 per hour will be paid for all hours worked after 10:00 PM Eastern Time.Weekend Differential: An additional $1.50 per hour will be paid for all hours worked on Saturdays and Sundays, regardless of time of day.Shift differential pay is calculated in addition to base hourly wages and will be reflected on the corresponding payroll cycle.Bonus potential: Earn up to $1,500 per month based on patient engagement and performance goalsKey ResponsibilitiesConduct outbound phone calls to check in on patients and address health concerns (expected call volume ranges from 70 to 90 calls per day)Handle inbound phone calls and route appropriately based on clinical urgencyRoute non-clinical inbound calls to the appropriate departments across the companyMonitor and respond to Remote Patient Monitoring (RPM) alerts, escalating concerns when clinically indicatedCollaborate with providers to coordinate timely and effective patient carePerform monthly wellness assessments and complete comprehensive chart reviewsAccurately document all patient interactions in our clinical platform in real timeConsistently meet or exceed individual and team performance metrics related to:Care qualityPatient engagementResponse timesAdherence to protocol standardsQualificationsActive LVN or RN license (required)At least 1 year of nursing experience preferred (RPM, telehealth, or chronic care experience a plus)Strong communication skills with the ability to build patient trust remotelyProficient with computers, EMRs, and telehealth toolsReliable, high-speed internet and a HIPAA-compliant home workspace (Internet speed test required during the interview process) A hardwired internet connection is required to ensure call clarityWhy Nsight Health?100% remote position — no commute requiredBe part of a supportive, patient-centered care teamHelp close gaps in care and make a measurable impact on patient outcomesCompetitive pay with shift differential and bonus potentialOpportunities to grow with a fast-paced, innovative healthcare companyComprehensive Benefits PackageSign-On Bonus: This role is eligible for a one-time $3,000 sign-on bonus.
