Key Responsibilities
 Assess and diagnose a range of behavioral health conditions, both chronic and acute.
 Collaborate with a multidisciplinary health team to develop personalized treatment plans tailored to individual member needs.
 Conduct initial intakes and deliver therapeutic interventions through individual, family, and group sessions.
 Facilitate access to community resources for comprehensive behavioral health support.
 Engage with primary care providers and specialists to monitor treatment progress and discuss care strategies.
 Provide psychoeducation to members, focusing on mental health promotion and preventive care.
 Nurture trusting relationships with patients, aligning care with their goals and values.
 Participate in multidisciplinary case conferences to ensure effective and holistic care.
 Document care and coordination efforts promptly in our electronic medical record (EMR) system.
Required Skills
 2+ years of outpatient experience with adolescents and adults enrolled in Medicaid, Medicare, or dual-eligible.
 3+ years of experience in assessing, planning, and managing behavioral health care through direct clinical experience.
 1+ year of telemedicine experience.
 Strong understanding of trauma-informed and evidence-based psychotherapy.
 Proficiency in electronic medical record documentation and a commitment to leveraging technology for improved care.
 Exceptional judgment in evaluating situations and making informed decisions.
 Effective collaboration skills with diverse healthcare professionals.
Qualifications
 Active LCSW, LMFT, or LCPC license is required.
 Candidates with MFT and CPC internships are also encouraged to apply.
