Job Overview

We invite you to become a part of our dynamic team as a Telephonic Nurse Care Manager. In this crucial role, you will assist our members in navigating intricate medical and behavioral health scenarios. You will work closely with a diverse multidisciplinary clinical team, which includes healthcare professionals, care coordinators, and records specialists, to provide compassionate and integrated remote care.

Key Responsibilities
• Deliver coordinated, patient-focused virtual care management via phone or video consultations aimed at enhancing members’ health outcomes.
• Collaborate effectively with members and the multidisciplinary care team to formulate impactful care plans that support the achievement of health objectives.
• Guide members through complex medical conditions, treatment options, benefits, and the healthcare system.
• Establish partnerships with local providers to ensure seamless care coordination.
• Provide compassionate, ongoing follow-up care, nurturing supportive relationships with members.
• Assist members and their families during critical healthcare events, including hospitalizations and rehabilitation, by offering coordinated case management.
• Integrate essential resources to comprehensively address members’ clinical and social needs.

Required Skills
• Bachelor of Science in Nursing (BSN).
• Current CCM certification is mandatory.
• Strong communication and advocacy capabilities.
• Proficiency in managing complex medical situations and navigating healthcare systems.
• Empathy and cultural competence when interacting with diverse populations.
• Technological proficiency and familiarity with various medical record systems.

Qualifications
• A minimum of five years of nursing experience is preferred.
• At least two years of experience in care management is required.
• Residency in a compact NLC state is essential.
• Active compact RN license in good standing with the nursing board of your state is required.
• Active California Nursing License is necessary.
• Willingness to obtain and maintain licensure across multiple states.
• Experience in remote work settings is preferred.
