Job Summary
We are seeking an experienced Physical Therapist (PT) to join our team, primarily covering the greater Mansfield and/or Norwich region, with opportunities also available towards the CT shoreline or Colchester area. As a Physical Therapist with Hartford HealthCare at Home, you will have the flexibility to choose from full-time, part-time, or per diem hours, with benefits eligibility starting at 24 hours per week or more.

Key Responsibilities and Duties

Evaluate, plan, and implement treatment for homebound patients referred for physical therapy
Independently evaluate, determine treatment strategies, deliver, and/or delegate services to identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain
Emphasize physical dysfunction, using evaluation skills and techniques, analysis of patients, and therapeutic application of physical and chemical agents, exercise, and other procedures to maximize the patient's functional independence
Conduct evaluations of strength, motor control, joint mobility, soft tissue integrity, pain, balance, coordination, posture, cardiopulmonary function, endurance, gait, and functional abilities in activities of daily living

Qualifications and Requirements
To be successful in this role, you will need:

PT experience, with a graduate degree from an accredited American Physical Therapy Association school (BS or entry-level master's degree)
An active CT PT license and CPR certification at the time of start
A strong passion for delivering high-quality patient care and a commitment to our organization's values and mission
