Hospital Liaison-Weekends-Remote Qualifications & Responsibilities...
Ā Foster collaborative relationships with hospital discharge planners and medical professionals.
Ā Conduct assessments and evaluations of potential residents in hospital settings.
Ā Coordinate smooth transitions for patients from hospitals to our nursing home.
Ā Educate healthcare providers and community partners about our facility's services and capabilities.
Ā Act as a liaison between the nursing home and external healthcare entities.
Ā Bachelor's degree in healthcare administration, business, or a related field (preferred).
Ā 3-5 years Previous experience as an Admissions Director or Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital desired.
Ā Strong communication and interpersonal skills.
Ā Knowledge of healthcare regulations and standards.
