We are seeking a dedicated and experienced Hospice Patient Care Coordinator to join our team at Intrepid USA, a leading national home healthcare company. As a Hospice Patient Care Coordinator, you will play a vital role in providing clinical and management support to our Clinical Nursing Supervisor, ensuring the delivery of high-quality, patient-centered care to our hospice patients and their families.
Key Responsibilities:
Provide clinical and management support to the Clinical Nursing Supervisor to ensure quality and safe delivery of hospice care services
Assist with staffing and scheduling, admissions, and nursing visits as needed
Facilitate communication and coordination of care between clients, families, caregivers, physicians, and other service providers
Assure efficient coordination of caregivers through case conferences and scheduling processes
Monitor patient records to ensure compliance with documentation requirements and program-specific rules
Participate in quality assurance and performance improvement activities
Provide orientation, training, and in-services to new hires and the care team

Requirements:
Graduate of an accredited school of nursing
Current Registered Nurse licensure (in good standing) in the state(s) of practice
Current CPR certification
Valid driver's license and automobile insurance
Three (3) years of clinical experience, including two (2) years of hospice experience and one (1) year of supervisory experience in a clinical setting
