Job Summary We are seeking a highly skilled and compassionate Hospice Medical Social Worker to join our team in Norfolk, NE. As a Hospice Medical Social Worker, you will play a vital role in our interdisciplinary team, providing essential support and care to patients and their families. This is a part-time, remote opportunity with a competitive salary range of $27-$38 per hour, depending on experience. Key Responsibilities Assess and address the psychosocial, emotional, and financial needs of patients and their families, providing support and guidance to help them cope with their situation. Develop and implement individualized plans of care, collaborating with the interdisciplinary team to ensure comprehensive care. Provide education and support to patients, families, and caregivers on medical social work interventions and treatment plans. Identify and address potential risks or threats to patient health, implementing interventions to mitigate these risks and promoting a safe environment. Communicate effectively with patients, families, caregivers, and healthcare team members, ensuring that changes in patient status are promptly reported and addressed. Perform care planning, transfer, and discharge functions, ensuring seamless transitions and continuity of care. Participate in ongoing professional development, staying up-to-date on best practices and regulatory requirements. Requirements and Qualifications Master's or doctoral degree in Social Work from a school accredited by the Council on Social Work Education. At least one year of experience in a healthcare setting, preferably in hospice or home health care. Current licensure as a social worker in the state of Nebraska. Current CPR certification. Strong communication and interpersonal skills, with the ability to work effectively with patients, families, caregivers, and healthcare team members. Ability to work independently and as part of an interdisciplinary team. Strong assessment and problem-solving skills, with the ability to prioritize tasks and manage multiple responsibilities. Preferred Qualifications Experience with electronic medical records and other healthcare software. Familiarity with hospice and home health care regulations and guidelines.
