Home Infusion Pharmacist Nashville, TN

We are seeking a skilled and patient-focused Pharmacist for a home infusion pharmacy located in Nashville, TN. This is a contract opportunity starting in late June or by July 1st, ideal for pharmacists with home infusion or clinical backgrounds looking to join a reputable nationwide infusion provider.

Position Details:
• Start Date: Late June / July 1st
• Schedule: MondayFriday, 8:30 AM 5:00 PM
• Rotates to 9:30 AM 6:00 PM shift when on-call
• Type: Full-time, Contract
• Setting: Home infusion (facility and home-based infusion services)

Key Responsibilities:
• Review and approve prescriptions prepared by pharmacy technicians in accordance with state regulations
• Perform final verification on filled prescriptions to ensure order accuracy and timely delivery
• Conduct clinical medication assessments using patient-specific medical data
• Collaborate with pharmacy leadership to ensure pliance with federal/state laws and pany policies, particularly regarding controlled substances
• Support quality assurance and cost containment initiatives related to drug procurement and dispensing
• Respond to clinical questions from internal staff (technicians, nurses) and external providers
• Assess laboratory results and therapeutic drug ranges to ensure optimal patient outes
• Provide leadership support in the absence of the Pharmacist in Charge
• Participate in the on-call rotation as needed
• Maintain strict pliance with HIPAA regulations and protect patient confidentiality

Requirements:
• Graduate of an accredited school of pharmacy
• Active pharmacist license in the state of Tennessee (or willingness to obtain)
• Prior home infusion or clinical pharmacy experience preferred
• Strong munication and multitasking skills
• Proficient puter and documentation abilities
• Ability to work effectively as part of a clinical and operational team
