Home Infusion Pharmacist Nashville, TN
Minimum 1+ years pharmacy experience required.
We are seeking a skilled and patient-focused Pharmacist for a home infusion pharmacy located in Nashville, TN. This is a contract opportunity starting in late June or by July 1st, ideal for pharmacists with home infusion or clinical backgrounds looking to join a reputable nationwide infusion provider.
Position Details:
ā¢ Start Date: Late June / July 1st
ā¢ Schedule: MondayFriday, 8:30 AM 5:00 PM
ā¢ Rotates to 9:30 AM 6:00 PM shift when on-call
ā¢ Type: Full-time, Contract
ā¢ Setting: Home infusion (facility and home-based infusion services)
Key Responsibilities:
ā¢ Review and approve prescriptions prepared by pharmacy technicians in accordance with state regulations
ā¢ Perform final verification on filled prescriptions to ensure order accuracy and timely delivery
ā¢ Conduct clinical medication assessments using patient-specific medical data
ā¢ Collaborate with pharmacy leadership to ensure pliance with federal/state laws and pany policies, particularly regarding controlled substances
ā¢ Support quality assurance and cost containment initiatives related to drug procurement and dispensing
ā¢ Respond to clinical questions from internal staff (technicians, nurses) and external providers
ā¢ Assess laboratory results and therapeutic drug ranges to ensure optimal patient outes
ā¢ Provide leadership support in the absence of the Pharmacist in Charge
ā¢ Participate in the on-call rotation as needed
ā¢ Maintain strict pliance with HIPAA regulations and protect patient confidentiality
Requirements:
ā¢ Graduate of an accredited school of pharmacy
ā¢ Active pharmacist license in the state of Tennessee (or willingness to obtain)
ā¢ Prior home infusion or clinical pharmacy experience preferred
ā¢ Strong munication and multitasking skills
ā¢ Proficient puter and documentation abilities
ā¢ Ability to work effectively as part of a clinical and operational team
