As a Home Health Weekend Intake & Scheduling Coordinator with h/care, you will have the unique opportunity to impact lives directly from the comfort of your home. Your role will primarily focus on managing the intake and scheduling logistics necessary for providing seamless home health care during weekends. This position offers you the flexibility to balance other responsibilities while playing a critical role in a loving and caring environment.

Duties and Responsibilities
• Coordinate with patients, families, and caregivers to facilitate home health service setup during weekends.
• Handle telecommunications efficiently by answering calls and responding to emails pertaining to home health care inquiries.
• Manage scheduling of healthcare professionals and adjust rosters based on patient needs and staff availability.
• Ensure all intake forms are accurately completed and processed in the organization's patient management systems.
• Collaborate with clinical staff to inform them of new intakes and any specific patient needs or requests.
• Conduct initial screenings of patient needs and align those with the available resources and healthcare solutions.
• Maintain complete confidentiality of all patient information, adhering to HIPAA regulations and company policies.
• Provide updates and report any significant changes in patient handling or scheduling patterns to supervisors.
• Study and understand healthcare service plans and possibly suggest necessary adjustments or variations.
• Participate in weekly team meetings to discuss scheduling challenges and opportunities for improvement.
• Use of technology in maintaining efficient and effective communication across all relevant platforms within the company.
• Experience with Careport beneficial.

Requirements
• Proven experience in intake coordination, scheduling, or relevant administrative positions within healthcare.
• High school diploma or equivalent required; additional certification in healthcare administration preferred.
• Strong organizational and multitasking abilities with a heavy emphasis on attention to detail and accuracy.
• Exceptional interpersonal and communication skills, capable of managing sensitive information with confidentiality and tact.
• Ability to work independently, managing a wide range of activities and tasks.
• Proficient in the use of modern communication tools and healthcare information systems.
• Adept at critical thinking and problem-solving in fast-paced environments.
• Comprehensive understanding of home health regulatory requirements and best practices.
• Willingness to work flexible hours specifically during weekends and holidays as required.
