Position Highlights
ā¢ Setting : Home Health
ā¢ Schedule : Monday-Friday, daytime; flexible options available; weekends off; autonomy to manage your visit schedule
ā¢ Location : San Francisco, CA (serving patients within a 5-10 mile territory radius)
ā¢ Population Served : Adult and geriatric home-based patients
ā¢ Compensation : $130,000-$190,000 per year (dependent on experience)

Key Responsibilities :
ā¢ Assess patient needs and create personalized home-based physical therapy treatment plans.
ā¢ Implement evidence-based interventions to enhance mobility and alleviate pain.
ā¢ Monitor patient progress and modify treatment plans for optimal outcomes.
ā¢ Educate patients and caregivers on safe exercise practices and mobility strategies.
ā¢ Collaborate with a multidisciplinary care team to ensure excellent patient outcomes.
ā¢ Maintain timely and accurate documentation using user-friendly digital systems.
ā¢ Promote independence and deliver compassionate, supportive care to patients.
Qualifications :
ā¢ Valid and unrestricted California PT license
ā¢ At least 1 year of Physical Therapy experience
ā¢ Current AHA CPR certification
ā¢ Valid Driver's License and reliable transportation required
ā¢ Excellent communication, time-management, and clinical reasoning skills
