Job Summary: Home Health Physical Therapist - Full Time We are seeking a highly skilled and compassionate Home Health Physical Therapist to join our team on a full-time basis. As a Home Health Physical Therapist , you will play a crucial role in providing high-quality patient care, promoting optimal physical function, and improving overall well-being. Key Responsibilities: Plan and administer prescribed skilled physical therapy treatment and training for patients with various injuries, illnesses, and functional disabilities to achieve the highest level of physical function. Evaluate patients' physical abilities, determine their potential to respond to therapy, and develop treatment plans. Test and screen patients' physical strengths to assist physicians in evaluating patients' levels of function and record findings to develop or pursue treatment programs. Develop and implement conditioning/rehabilitation programs consistent with physicians' Plans of Treatment and overall patient/rehab team goals. Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising therapy treatment programs and Plans of Treatment. Provide Physical Therapy Assistants and Home Health Aide staff with written instructions/care plans, supervise and evaluate staff performance. Monitor the completion of documentation by physical therapy assistants and home health aides/personal care workers. Accurately, promptly, and thoroughly document patients' care observations, interventions, and evaluations. Report patients' progress to physicians, Clinical Managers, staff, patients, and families. Submit evaluation, treatment plans, progress reports, and discharge summaries to supervisors and care management staff. Requirements and Qualifications: Essential Qualifications: Degree from an accredited Physical Therapy Program (approved by the APTA). Current and unrestricted Physical Therapy license. Current CPR certification. Valid driver’s license, auto insurance, and reliable transportation. Strong organizational and communication skills. Preferred Qualifications: Minimum of one year of physical therapy experience. Skills and Competencies: Excellent communication and interpersonal skills. Strong critical thinking and problem-solving abilities. Ability to work independently and as part of a multi-disciplinary team. Compassionate and patient-centered approach to care. Compensation and Benefits: Our compensation range for this position is $101,400 - $139,575 per year, depending on experience and qualifications.
